Keep referrals, reviews and service counts together
Coordinating eating disorder Medicare care means keeping several things separate: the plan period, services remaining on a referral, overall psychological service use and required medical reviews.
The Eating Disorder Medicare Coordination Pack gives psychologists and practice staff editable records, communication templates and an Excel companion for organising that information.
Use it to record what has been checked, identify outstanding tasks and make the next follow-up clear.
Two files included
1. Editable Word pack — 13 pages
The Word document includes:
- Getting-started guidance: How to adapt the pack and use it within your practice.
- Quick-reference table: Distinguishes an exhausted referral, expired plan, overall cap reached and remaining referral balance.
- Team and consent record: Record relevant practitioners, secure contacts, information-sharing arrangements and follow-up responsibilities.
- Numbered psychological service tracker: Forty record positions across two pages, with milestone shading and evidence-status dropdowns.
- Medical review record: Track managing-practitioner reviews and distinguish specialist referral, booking, completion, outcome and claim confirmation.
- Expiry checklist: Organise advance reminders, new-plan arrangements, referrals and continuity-of-care discussions.
- Progress-letter template: A short clinical update with a five-line coordination facts block.
- Client communication templates: An adaptable email and scripts for referral limits, approaching expiry and information still being checked.
- Three fictional worked examples: Specialist-review coordination, expiry with unused services and Better Access overlap.
- Official source links, local review fields, changelog and licence.
Word records are manually completed. Editable fields and dropdowns support entry; they do not verify the information entered.
2. Excel coordination companion
The Excel workbook contains three tabs:
- Tracker: A summary of entered dates, recorded service use, referral arithmetic and follow-up prompts.
- Services: A numbered register with pathway and evidence-status dropdowns, running counts and incomplete-entry flags.
- Guide: Instructions, calculation explanations, limitations and official source links.
Based on your entries, the workbook calculates:
- The plan’s 12-month anniversary and days remaining.
- Recorded and verified psychological service counts.
- Use against the selected eating disorder referral.
- An arithmetic referral balance.
- Upcoming eating disorder review checkpoints.
- Prompts for approaching expiry, incomplete information and dates outside the entered plan period.
Suggested reminder settings are editable. The workbook does not send email or calendar reminders.
Who it’s for
Psychologists, practice owners and administrative staff coordinating care under an Eating Disorder Treatment and Management Plan (EDTMP).
Use one client record and one workbook per client and plan period. Store completed files securely within your authorised practice systems.
Before use
Adapt the templates to your practice and check current official requirements.
This is a coordination resource. It does not verify Medicare eligibility, connect to Medicare systems, submit claims, provide clinical decision support or guarantee payment or compliance.
The workbook calculates a 12-month anniversary, not a verified last claimable day. A recorded balance does not confirm that another service can be claimed.
Independent clinical or billing review has not been completed.
Created by Ethan Smith for MindFrame, published through PsychVault. Not an official or endorsed Medicare or Services Australia resource.
Version 1.1, updated 2 November 2026. Suggested review date: 25 January 2027. Automatic updates and buyer notifications are not included.
